SHS 121 - Case Management and Community Resources
3.0 Credits Defines the role and functions of a case manager, history, challenges of the job, and knowledge and skills necessary to be effective including how to make referrals and act as an advocate and community resource specialist.
Course Objectives Upon successful completion of this course, students will be able to:
- Define the concept “Case Management.” [REASON]
- Define tasks or functions of case manager. [REASON]
- Identify key components of a Strengths Based Case Management approach/style. [REASON]
- Identify what kinds of knowledge and skills are necessary to effectively perform the case manager role. [REASON]
- Identify the essential elements of a comprehensive individualized service or personal responsibility plan (also referred to as a Case Plan). [EXPLORE]
- Identify a basic process for resolving potential or real ethical dilemmas involving service delivery. [EXPLORE]
